To help expedite our investigation, please upload photos of the following:
- Your valid photo ID (driver’s license, passport, state ID or Permanent Resident Card / Green Card)
- Photo of yourself holding your valid photo ID
- Any additional optional documents that you’d like to provide to help us better understand the situation (police report, court findings, other applicable documentation)
When uploading documents:
- Remove all documents from cases or holders
- Snap a picture of the document and upload it below
- Make sure the image is clear, all four corners of the document are visible, and all text is legible before submitting
Documents that are photocopied, altered, counterfeit, torn, laminated, erased or redacted will not be accepted.
As a potential victim of identity theft, we recommend you take the following steps:
- File an identity theft report with the IRS. More info is on the IRS website.
- Report the incident to the FTC by completing this online form or call 1-877-438-4338, and file your police report using a copy of your FTC identity theft report.
If you believe your Social Security number has been compromised, we recommend contacting the Social Security Administration or reviewing their identity theft resources.
