Sending documents to the TLC

The TLC requires license applicants to upload copies of the following three documents:

• Driver’s license (Class E, CDL, NJ, CT, or PA) plus a certified abstract if your license is from outside of New York State
• Defensive driving certificate
• Completed medical examination form

Upload your required documents using the** TLC Upload Portal **

You can use TLC Upload Portal to look into the status of items you have submitted to the TLC, additional information on your license application, and important information on appointments or documents the TLC may have sent you. The TLC Upload Portal is available 24 hours a day, 7 days a week, to check on the status of an application.
