If you need a new LAX placard because you’ve lost yours or haven’t received one after completing the LAX quiz, follow these steps:
Click the link provided to request a new placard by going to get a new placard.
Make sure your address is current in your driver profile. Update it if necessary before you submit the request form.
We’ll send the placard to your address on file. Please allow 5-7 business days for it to arrive.
If you passed the LAX quiz less than two weeks ago, your placard might already be on its way to you.
