If you need a new LAX placard because you’ve lost yours or haven’t received one after completing the LAX quiz, follow these steps:

  • Click the link provided to request a new placard by going to get a new placard.

  • Make sure your address is current in your driver profile. Update it if necessary before you submit the request form.

  • We’ll send the placard to your address on file. Please allow 5-7 business days for it to arrive.

If you passed the LAX quiz less than two weeks ago, your placard might already be on its way to you.
