- FTC ID Theft Report receipt for US earners. Ensuring Federal Authorities are made aware of the issue.
- Additional Optional Documents that you’d like to provide to help us better understand the situation (police report, court findings, other applicable documentation)
When uploading documents:
- Remove all documents from cases or holders
- Snap a picture of the document and upload it below
- Make sure the image is clear, all four corners of the document are visible, and all text is legible before submitting
Documents that are photocopied, altered, counterfeit, torn, laminated, erased or redacted will not be accepted.
