The purpose of this form is to connect you with your Health and Safety Representative (HSR) and/or Delivery Person Representative (DPR) about feedback regarding health or safety. This can include providing input or feedback about:

Incidents that you were involved in such as road safety or verbal and physical abuse
Near-miss safety incidents, even ones where you have not been injured
Safety issues with deliveries, such as items being too heavy or bulky

Please note that we may contact you to gather additional information, as well as your contact details, in order to forward your feedback to your representative.

Contacting your HSR/DPR does not replace any existing safety processes or resources.
